Create An Input Form
Click the Forms Tab 
The Grocery Sales query disappears.
Click 
Access displays the New Form window which provides you with several
different
Form Wizards
to create a form. You can also create a new form without a wizard in design view.
The lower portion of the Form window allows you to specify the
data object.
